Where are the Condition names and values stored within the model?

I need to link the condition name from the model to a query over a data file. Does anyone know what file in the model contains the condition name and the condition values? For example, Field1 has conditions set up as COND1 and COND2, with the name of COND1 = This is Condition 1, and COND2 = This is Condition 2. What file can be used to match the condition value, ie COND1, with the condtion name, ie 'This is Condition 1'?
